How to Change Name in ESIC - अगर ESI में नाम गलत है तो कैसे ठीक करें

 

How to Correct Name in ESIC

1.   You should have your IP number with you. It may be printed on your Pay slip or you can get it from your Company.

 

2. Also, you have to carry your TIC (Temporary Insurance Card) with your full detail.

 

 

3. A letter containing detail your correct & wrong detail & duly signed by you.

 

4. Letter should be on Company letter head & Attested by Employer.

 

 

5. Attach copy of your Aadhaar & TIC with this Form duly signed by you & attested by Employer.

 

6. Submit the Letter along with Documents to respective ESI office.
